What counts as an environmental modification
- Wheelchair ramps, landings and handrails.
- Accessible bathrooms: roll-in or walk-in showers, tub-to-shower conversions, grab bars, comfort-height toilets and anti-scald valves.
- Stair lifts and porch (vertical platform) lifts.
- Doorway widening, offset hinges and level thresholds.
- Slip-resistant flooring and safer stairs.
- Safety fixes named in the evaluation, such as protected wiring or durable wall surfaces.
- Programs call this work by different names: environmental modifications (EMOD), environmental accessibility adaptations, home modifications or residential modifications. It's the same idea: changes to the home that let a person live there safely and more independently.
New Jersey programs that pay for EMODs
- NJ DDD (Division of Developmental Disabilities): pays for approved environmental modifications for people enrolled in the Supports Program or Community Care Program, through its payment agency Acumen. We are an approved Acumen vendor (#312504). The request starts with the person's support coordinator.
- NJ FamilyCare long-term care (MLTSS): the managed care plans (Aetna Better Health, Fidelis Care, Horizon NJ Health, UnitedHealthcare Community Plan and Wellpoint) can approve residential modifications. The plan's care manager makes the request.
- JACC (Jersey Assistance for Community Caregiving): for adults 60 and over who don't have Medicaid. It can include environmental accessibility adaptations. Apply through the county office on aging.
- Children's System of Care (PerformCare): Family Support Services for youth under 21 with intellectual or developmental disabilities can include home (environmental) modifications.
- Catastrophic Illness in Children Relief Fund: can reimburse families for completed home modifications for a child, up to $25,000.
- NJ TBI Fund: can pay for environmental modifications for people living with a brain injury.
- NJ DVRS (Vocational Rehabilitation): sometimes funds home access needed to get to work.
- Veterans: VA HISA, SAH and SHA grants.

How the EMOD process works
- 1. The support coordinator, care manager or family asks the program for a home modification.
- 2. An evaluator (often an OT or PT) visits and writes down what the person needs.
- 3. We do a free home visit and write an itemized quote that matches the evaluation and the program's rules.
- 4. The program reviews and approves it. In NJ DDD this is the prior authorization (SDR) through Acumen.
- 5. We pull any permits, build the approved work, and keep the coordinator updated with photos.
- 6. Final walk-through with the family, then we bill the program directly. For approved work, families don't pay us.

Questions people ask
What is an environmental modification (EMOD)?
A change to the home, such as a ramp, accessible bathroom, lift or wider doorway, that helps a person with a disability or an older adult live there safely and more independently. Medicaid waivers and state programs often pay for them.
Does NJ DDD pay for environmental modifications?
Yes. People enrolled in the NJ DDD Supports Program or Community Care Program can get approved environmental modifications, paid through Acumen. Ask your support coordinator to start the request. We're an approved Acumen vendor.
Do families pay anything?
For work the program approves, the program pays us directly. If you want extras the program won't cover, we quote those separately in writing first.
How long does an EMOD take in New Jersey?
The approval steps usually take longer than the work. Once approved, most ramps and bathroom changes take days to a couple of weeks, depending on materials and permits.
Can you revise a quote the program sent back?
Yes. Programs often ask for changes to match their guidelines. We revise and resubmit quickly, and keep the coordinator in the loop.
